The main difference is that Medieval thinking was preoccupied with God and religion, whereas Humanism is pre-occupied with reason, progress and science. If I had to put it in just one sentence, that would be it.

Humanism believes that reason is the ultimate virtue, whereas the Medieval's thought that the eternal was the ultimate virtue (such as God). Humanism believes in progress and science, which is that we continually improve society and mankind and are progressing towards a utopian society, or at least a better society, through our use of reason and through improvements in technology.

The Enlightenment was the birth of Humanism as we perceive it today.
